OOPS - PHYSICAL ASSESSMENT
SHOULDER

Mobirise

See instructions for drawing your pain on devices.

You can draw a pain chart / graph to show how the pain changes over the course of 24 hours.

Then follow the range of movement guide below. It might be useful to have another person to estimate how much of your range you have before you feel pain or stiffness.

Write the percentage of movements in the calculator below and it will give you the average of all ranges as your functional range. This will give you the starting point and link for your rehabilitation as below.
Write this down so you can refer to it every few days to measure your improvement by following the rehabilitation guides as below. Then click the clear button to clear the form for the next time you use.

Posture
Check your posture in the mirror front on. Does one shoulder seem lower to the other? Is it possible to level up? Write a note. Remember, you are looking at a mirror image.

Write it like: Posture = (R)right shoulder is lower than left.

Have someone check your posture from behind. Do the shoulder blades sit flat or is it 'winging'? Write a note.
Flexion
Lift your arm with a straight elbow, up in front as far as you can.

By side = 0%
Straight out at right angle = 50%
Straight up beside ear = 100%

Write it as F = e.g. (33%)
Abduction
Lift your arm with straight elbow, out to the side, turning the hand outward and then up, as far as you can.
Note if there is pain part way that then ceases as you go higher. Use the painful 'catch' as the measurement.

By side = 0%
Straight out at right angle = 50%
Straight up by ear = 100%

Write it as Abd (Left) = e.g (50%)
Hand Behind Back
Take your hand behind your back and up as far as it can.

Hand to hip = 25%
Hand to low back = 50%
Hand to opposite lower ribs = 75%
Touching the lower corner of the opposite shoulder blade = 100%.

Write it as HBB = e.g. (50%)
External Rotation
With your arm by your side, bend your elbow. Now take your hand outward as far as it goes.

20 degrees = 25%
40 degrees = 50%
60 degrees = 75%
80 degrees or nearly straight out right angle from the body = 100%

Write is as HBB = e.g.(66%)
